The collective bargaining group provides a variety of forms to its members. These forms include union membership applications, CMPTEC benefit forms, grievance forms, and more. All forms are designed to ensure the rights and benefits of members are protected and to streamline the collective bargaining process. Forms are available in digital pdf format and can be accessed online below. CMPTEC board members can aid members in filling out forms if they require assistance.
